HOME CARE LPN OFFICE NURSE

Job Description:

• Provides direct patient care, to include using a variety of skills and necessary coordination of services. - - - -
• Completes appropriate documentation as required by policy and procedure and regulation. - - - -
• Assists Patient Care Coordinator and/or Operations Manager in day to day branch office tasks to ensure patient care documentation is complete and submitted timely. - - - -
• Assists Patient Care Coordinator and/or Operations Manager in management of patient care supplies. - - - -
• Manages all commercial insurance payers and Medicare replacement plans for continued authorization regarding home care visits and documents authorization in electronic medical record per software system. - - - -
• Coordinates with Operations Manager and Patient Care Coordinator in the management of Face to Face documentation as required by specific payer source.Job Requirements:

  • Current LPN licensure within the state of branch location, and successful completion of nursing education from an approved school of nursing. - - - -
  • One year of clinical nursing experience, preferably in an acute care medical/surgical setting. - - - -
  • One year of Home Care experience preferred. - - - -
  • Ability to independently organize work and provide effective health care services to homebound patients. - - - -
  • Effective communication skills (oral and written). - - - -
